On the Application of Medication Therapy Management Mode in Multidisciplinary Team for Anti-infection Diagnosis and Treatment

Abstract

Objective: To explore the establishment of multi-disciplinary team (MDT) for anti-infection diagnosis and treatment mode for pharmacists and to provide references for pharmacists to participate in the MDT. Methods: The medication therapy management (MTM) mode of the United States was referred to. Five key elements, including collection of patient information, medication therapy review, medication action plan, intervention and follow-up, were combined with the real cases of our hospital to discuss the establishment of standard pharmaceutical service process. Results: The establishment of the service process has improved the influence of pharmacists in MDT and provided references for the establishment of the pharmacists' performance appraisal system at the present stage. Conclusion: The MTM mode is helpful to establishing the standard of multidisciplinary team for anti-infection diagnosis and treatment, but more research data are needed to optimize the development of standards for medication-related problems.
