目的：研究基本药物地塞米松在治疗呼吸道感染疾病中的临床用药情况并分析其原因。方法：对开封某三甲医院呼吸道感染疾病641例处方进行统计分析，查阅大量文献，与其他非基本药物作对比分析。结果：在呼吸道感染疾病处方中，基本药物地塞米松使用率低于非基本药物克林霉素磷酸酯。结论：虽然地塞米松价格低廉，临床治疗呼吸道感染疾病效果显著，但因其严重的不良反应和较低的利润空间，临床使用率低于非基本药物克林霉素磷酸酯。同时，因克林霉素磷酸酯是抗菌药物，并不是治疗由病毒引起的上呼吸道感染疾病的首选用药，因此，该院可能存在抗生素使用不合理的情况。
Objective: To analyze the clinical application of essential drug dexamethasone in the treatment of respiratory tract infection and its causes. Methods: The prescriptions for 641 cases of respiratory tract infection of tertiary frst-class hospital of Kaifeng were analyzed and compared with other non-essential drugs under the guidance of many literatures. Resuults: The essential drug dexamethasone was less frequently used than nonessential drug clindamycin phosphate in the prescriptions for respiratory tract infection. Conclusion: Although dexamethasone was inexpensive and the clinical treatment effect was signifcant, its clinical use rate was lower than that of clindamycin phosphate because of its serious adverse reactions and low proft margins. Moreover, clindamycin phosphate was an antibiotic and was not the preferred drugs to treat upper respiratory tract infection caused by virus. Therefore, there was an irrational use of antibiotics in the hospital.
